PLEASE READ THE FOLLOWING INFORMATION BEFORE SUBMITTING THIS FORM.
Our Mission is to create an environment of belonging and safety where individuals of diverse ages, races, cultures and backgrounds can fellowship, nurture, support and counsel in a caring, participative community.

-> Request forms must be completed three (3) weeks prior to use. Must be 18 years old.

-> Fees must be paid for use of Kitchen prior to use, to be determined.

-> Training for two people from requesting ministry must complete training on use of equipment prior to use; minimum of two people from requesting ministry.

-> A member of the Culinary/Fellowship ministry must be present at all times when Kitchen is in use. No exceptions.

-> Enclosed shoes must be worn when working in the kitchen area at all times. No open toe shoes allowed. No exceptions.

-> No open food items or personal supplies, tools, equipment, etc. can be left in Kitchen or Rhonda's Bistro.

-> All garbage must be carried out to dumpster for proper disposal at end of function.

-> At end of function, check list must be completed by requesting ministry.
